ABOUT THE JOB
The Behavioral Health Provider (BHP) is responsible for providing integrated behavioral health services to patients; especially under-served, uninsured, under-insured, high-risk and vulnerable patient populations residing in the service area regardless of their ability to pay for services. These duties include serving as a direct provider of behavioral health services providing quality, compassionate, and culturally sensitive care for patients according to evidence-based best practices for integrated behavioral health care. The BHP supports the care teams in coordination of patient care by provider or case manager referral and shared visits.
